Reproductive Health

Reproductive health refers to a total well-being in all aspects of reproduction - physical, emotional, behavioural and social. According to WHO, a reproductively healthy society has people with physically and functionally normal reproductive organs and normal emotional and behavioural interactions in all sex-related aspects.

Family Planning

India was the first country to launch a national family planning programme in 1951. The Reproductive and Child Health Care (RCH) programme aims at creating awareness about reproduction-related aspects and providing facilities for building a reproductively healthy society.

Contraception

Contraceptive methods include natural methods (periodic abstinence, coitus interruptus, lactational amenorrhea), barrier methods (condoms, diaphragm, cervical caps), IUDs (Lippes loop, CuT, Progestasert), oral pills (combined estrogen-progesterone; Saheli), injectables (DMPA), implants (Norplant), and emergency contraceptives. Condoms also protect against STIs.

MTP and Amniocentesis

Medical Termination of Pregnancy (MTP) has been legal in India since 1971. It is safe up to 12 weeks. Amniocentesis detects chromosomal abnormalities but is banned for sex determination under the PCPNDT Act.

STIs

Sexually transmitted infections include gonorrhoea (Neisseria gonorrhoeae), syphilis (Treponema pallidum), chlamydia, genital warts (HPV), genital herpes (HSV-2), HIV/AIDS, trichomoniasis, and hepatitis B. Early detection and treatment are essential.

Assisted Reproductive Technologies

ART includes IUI, IVF-ET, ZIFT, GIFT, ICSI, and surrogacy. IVF involves fertilization outside the body; the embryo at the 8-cell stage is transferred into the uterus (IUT) or fallopian tube (ZIFT). GIFT transfers the ovum and sperm into the fallopian tube. ICSI involves injecting a single sperm directly into the ovum.

Contraceptive methods span a spectrum from temporary/natural to permanent/surgical; condoms stand out as the only method offering STI protection alongside contraception.

  • Natural/traditional methods: periodic abstinence (avoiding coitus during the fertile period, roughly days 10 to 17 of the cycle), coitus interruptus (withdrawal before ejaculation), and lactational amenorrhea (absence of ovulation/menstruation during intense breastfeeding for up to 6 months postpartum)
  • Barrier methods: physically prevent sperm-egg contact; condoms (for both male and female), diaphragms, cervical caps, and vaults, often used with spermicidal creams/jellies/foams for higher effectiveness; condoms are the only contraceptive that also protects against STIs
  • Intra-uterine devices (IUDs): inserted by doctors into the uterus; non-medicated (Lippes loop), copper-releasing (CuT, Cu7, which increase phagocytosis of sperm and are also used as emergency contraceptives), and hormone-releasing IUDs (Progestasert, LNG-20) that make the uterus unsuitable for implantation and cervix hostile to sperm
  • Hormonal methods: oral pills (combination of estrogen and progestogen, or progestogen alone) inhibit ovulation and alter cervical mucus; can be taken as pills, injections, or implants under the skin
  • Surgical methods (sterilisation): highly effective, generally irreversible; vasectomy in males (vas deferens is cut and tied/sealed) and tubectomy in females (fallopian tubes are cut and tied/sealed) to block gamete transport

Infertility and Assisted Reproductive Technology (ART)

  • Infertility: inability to conceive even after one year of unprotected coitus; causes include physical, congenital, diseases, drugs, immunological, or even psychological problems in either partner
  • IVF (in vitro fertilisation), popularly called the "test tube baby" programme: ova from the wife/donor and sperm from the husband/donor are collected and induced to fuse in a laboratory dish to form a zygote; the zygote/early embryo (up to 8 blastomeres) is then transferred into the fallopian tube (ZIFT, zygote intra-fallopian transfer) or a later stage embryo is transferred into the uterus (IUT, intra-uterine transfer)
  • GIFT (gamete intra-fallopian transfer): transfer of an ovum collected from a donor into the fallopian tube of another female who cannot produce her own ova but can provide a suitable uterine environment
  • ICSI (intracytoplasmic sperm injection): a sperm is directly injected into the ovum to induce fertilisation in the laboratory, useful when sperm count/motility is very low
  • Artificial insemination (AI): semen collected from the husband or a healthy donor is artificially introduced into the vagina or uterus, used when the male partner has very low sperm count or impotency

Reproductive Health Problems and Strategies

  • Strategies for a reproductively healthy society: awareness and sex education in schools, information about reproductive organs, adolescence and related changes, safe and hygienic sexual practices, and awareness of STIs and AIDS
  • Support for a statutory ban on amniocentesis for sex-determination (to curb female foeticide), encouragement of small families, and medical assistance for pregnancy, delivery, contraception and infertility
  • Improved reproductive health is reflected in better detection and treatment of STIs, more assisted (medically supervised) births, better maternal and infant care, decreased maternal and infant mortality, and improved detection and cure of infertility

Population Explosion and Birth Control

  • Better health facilities and living conditions lowered death, maternal and infant mortality rates, causing a rapid rise (explosion) in population; the Reproductive and Child Health Care (RCH) programmes promote awareness and reproductive health
  • Measures to check population growth: motivating smaller families through contraception, raising the marriageable age (18 years for females, 21 years for males), and incentives to couples with small families
  • Ideal contraceptive: user-friendly, easily available, effective and reversible, with no or least side-effects, and not interfering with the sexual drive or act

  • Hepatitis B, genital herpes and HIV infections are not completely curable; the others can be cured if detected early and treated properly
  • Prevention: avoid sex with unknown or multiple partners, always use condoms, and seek early diagnosis and full treatment if infected

Medical Termination of Pregnancy (MTP)

  • MTP (induced abortion) is the intentional or voluntary termination of pregnancy before full term; legalised in India in 1971 with strict conditions to check misuse
  • Considered relatively safe during the first trimester (up to 12 weeks); second-trimester abortions are far riskier
  • Genuine uses include removing unwanted pregnancies (from casual relations, contraceptive failure or rape) and cases where continuing the pregnancy could harm the mother or the fetus
  • A serious concern is the misuse of MTP for illegal female foeticide following sex-determination — hence the statutory ban on prenatal sex-determination

Amniocentesis: A Prenatal Diagnostic Technique

  • Amniocentesis is a prenatal diagnostic test in which a sample of the amniotic fluid surrounding the developing foetus is withdrawn, and the foetal cells and dissolved substances in it are analysed
  • By studying the foetal chromosomes (karyotyping) it can detect chromosomal abnormalities and certain genetic/metabolic disorders (e.g. Down syndrome, haemophilia, sickle-cell anaemia) as well as the sex of the foetus
  • Misuse: because it can reveal foetal sex, amniocentesis has been misused for sex-determination followed by selective female foeticide; its use for disclosing foetal sex is legally banned in India under the PCPNDT Act, although the technique itself remains legitimate for detecting genetic disorders

Emergency Contraception

  • Emergency contraceptives are used to avoid a possible pregnancy after unprotected intercourse or after accidents such as contraceptive failure or rape
  • Administration of progestogens or progestogen-oestrogen combinations, or insertion of a copper-releasing IUD, within a short window (up to about 72 hours) of coitus can be quite effective as emergency contraception
  • By preventing an unwanted pregnancy, emergency contraception also reduces the need for induced abortion (MTP)

STIs: Symptoms, Complications and High-Risk Groups

  • Early symptoms of STIs are often minor or even absent; where present they may include itching, fluid discharge, slight pain, and swellings in the genital region, so infected persons are frequently unaware and reluctant to seek timely treatment
  • If left untreated, STIs can cause serious complications such as pelvic inflammatory disease (PID), infertility, ectopic pregnancies, abortions, stillbirths, and even cancer of the reproductive tract
  • STIs are especially common in the sexually active 15 to 24 year age group, which is why targeted awareness, safe practices, and early diagnosis matter most for this group
